  Parent/Guardians, Complete the 2016-17 Registration Process here.

Please read  and follow these steps carefully to be fully registered.  IMPORTANT:  Student-athletes will not be cleared to participate until ALL steps are completed.
Step 1: Student-Athletes must have a current sports qualifying physical exam on file in the Athletic Department Office.  Physicals are good for 13 months from exact date of your last physical. 

Once you have your physical completed by a doctor, you can turn it in to the office or you can scan it and submit completed Physical Forms online.


Step 2:  Review the Parent/Student-Athlete Handbook carefully.

Step 3:  View the Parent/Student-Athlete Presentation video in its entirety. 

Step 4:  Fill out and submit the Online Registration Form here (includes all forms in one).

Step 5:  Submit User Fee of $100 per student, per sport. Fees are due prior to the start of each season. For payment and waiver information, Click Here.

Baseline Concussion Testing 2016-2017

All Juniors,  Freshman and any new incoming athletes are required to take this prior to participation.
Sports that require this: 
Fall Sports: Football, Cheering, Field Hockey, Soccer, Diving.
Winter Sports: Ice hockey, Wrestling, Basketball, Diving, Gymnastics.
Spring Sports:  Lacrosse

Testing Date

March 16, 2017 - Spring Athletes (rescheduled from March 14)
2:15pm to 4:00pm
M-Wing Computer Lab (M-22)

​All testing will be done at Haverhill High School. Testing will be done in half hour sessions. First come first served. ​
